  • how she/he works with individuals with disabilities

  • the types of disabilities seen

  • what measures are important for treatment, psychological support and other factors that impact the adjustment

  • What is important for the case manager to know about the adjustment to patients’ disabilities and psychological support needs

  • Include a description of the facility, staff and work involved
